Yuji Itadori has a red hood because Gojo made a customized request for the boy’s uniform. He never said why, but Gojo personally went out of his way to add the hood to Yuji’s everyday school outfit.
He brings up his red hood for the first time in chapter #4. Both Yuji and Megumi are talking about their uniforms while waiting to meet their classmate Nobara for the first time.

At the start of chapter #4, Yuji Itadori and Megumi Fushiguro head to downtown Tokyo to meet their classmate Nobara Kugisaki for the first time.
Yuji remarks that only 3 first-year students seems a little small for class. Megumi asks if he has ever met anyone who can see curses, and Yuji says he guesses not.
Gojo then walks up to the two. He mentions to Yuji that it appears as though the boy’s school uniform has come on time. Yuji says it is a perfect fit, but it is different from Megumi’s uniform because it has a hood.
“School uniforms can be customized on request,” Gojo said. “I went ahead and asked for you.”
Yuji is a bit confused by this, but says it works out because he likes the hoodie anyways. Megumi reminds Yuji to be careful, as Gojo can sometimes act quirky like that.
